POSITION TITLE: Program Associate
The mission of CN Guidance & Counseling Services, Inc. is to provide clinical treatment, rehabilitation, housing opportunities, social and support services, counseling and guidance to individuals, families and the community affected by mental illness, developmental disabilities, psychological difficulties, addiction and/or dependency problems.
CN (Central Nassau) Guidance and Counseling Services has grown substantially since its inception. Our evolution has been shaped largely by the emerging needs of consumers as they move along the process of recovery.
JOB DEFINITION :
Facilitate and oversee all evidence-based workshops for individuals and couples. Lead and support outreach efforts to recruit and engage clients into the program. Perform continuous quality improvement by monitoring client records, ensure clients are receiving appropriate services and assisting other program staff in coordinating client services.
EDUCATION REQUIRED:
Bachelors Degree required (Masters preferred) with experience in healthcare/social services field
EXPERIENCE REQUIRED:
Experience in a mental health or related field
Experience working with at-risk youth
Experience in facilitating group-level
Bilingual in English and Spanish required.
DUTIES AND RESPONSIBILITIES:
Conduct workshops with fidelity to the evidence based curriculums, with schedule flexibility to provide such workshops on nights and weekends as needed by the participants.
Conduct outreach to recruit and engage potential clients and community organizations that serve the target population, including presentations to community providers and government entities.
Conduct intakes for potential participants in cooperation with Director both in the office and in the community.
Conduct survey assessments as required by the grant
Ensure that participants receive services and training appropriate to their needs and assist staff in maintaining consistent and professional collateral contacts for coordination of care.
Monitor all participant records to ensure documentations is in accordance with program and agency policies and standards.
Provide supports to participants around workshop content as needed both in the office and in the community.
Assist Administrative Assistant with CQI tasks related to the program.
Assist Evaluation Team with research elements of the program and data collection in relation to the local and federal evaluations.
Complete program documentation of participants? progress in both client record and electronic health records.
Coordinate with program partners to ensure all aspects of the program are conducted with fidelity.
Participate in team meetings as scheduled and partner meetings as needed.
Promptly report all emergencies, incidents, and maintenance and safety issues to the Program Director.
Additional duties as assigned.
