Registered Nurse (RN) Job Opening in Grand Junction, Colorado - Experienced home health Nurses (registered nurse or associates degree in nursing) are needed for Grand Junction and Mesa County areas! Responsibilities include (but are not limited to):
Provides skilled nursing care in accordance with the patient's plan of treatment, including comprehensive health and psycho-social evaluation, monitoring of the patient?s condition, health promotion and prevention, coordination of service, teaching and training activities, provision of direct nursing care and planning for patient?s needs or service requests.
Initial and ongoing comprehensive assessments (including OASIS data elements for home health)
Problem identification and goal setting
Initiates the plan of care and revises as necessary in consultation with the physician
Provides those services requiring substantial and specialized nursing skill.
Initiates appropriate preventive and rehabilitative nursing procedures.
Serves as coordinator of patient's care including referral to other services as appropriate and adherence to the plan of care.
Informs the physician and other personnel of changes in the patient's needs or service requests.
Teaches, supervises, and counsels the patient and family members regarding the disease process, self-care techniques, prevention strategies, and other related problems of the patient. Promotes patient/family independence.
Requisition and management of equipment and supplies appropriate to care.
Practice of standard precautions
Attends and participates in staff, education, and agency committee meetings as appropriate;
Assigns, supervises and demonstrates appropriate procedures as necessary to LPN?s and aides as required by policy and state regulations.
Maintains adequate and timely records and reports of evaluation data, interventions, and patient?s response to interventions.
Evaluates the effectiveness and outcomes of care.
Plans for the patient's discharge from service.
Participation in quality/performance improvement measures.
Participates in in-service programs.
Performs other related functions as required.
Level of Responsibility :
Works under supervision of Director of Services or designee.
Exercises supervision of assigned personnel including licensed practical nurses, home health aides, personal care providers and office support staff.
