JOB SUMMARY:
Coordinates treatment to provide comprehensive wound assessments and plan of care including goals, implementation of treatment, evaluation and outcomes for Wound Care Center patients. Provides patient, family and staff education regarding wound management, prevention of pressure ulcers, support surfaces, dressing selections and other therapies. The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served in the wound care clinic. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide the care needed as described in the unit's/area's/department's policies and procedures.
AGES SPECIFIC TO HOME HEALTH/HOSPICE:
13 years through 17 years
Young Adult- 18 years through 34 years
Middle Adult- 35 years through 50 years
Mature Adult- 51 years through 69 years
School Age- 6 years through 12 years
Geriatric Adult- 70 years +

ETHICS AND COMPLIANCE:
Employee performs within the prescribed limits of the Ethics and Compliance Program. Is responsible to detect, observe and report compliance variances to their immediate supervisor, or upward through the chain of command, the Compliance officer or hospital
ESSENTIAL FUNCTIONS:
1. Ability to perform wound assessment, reassessment and management for all patients and per policy. This includes pediatric, geriatric, and the general population.
2. Utilizes principles of current wound management theory when providing care to patients with acute and chronic wounds.
3. Demonstrates thorough knowledge of staging system of wounds.
4. Provides case management in the Wound Care Clinic to provide specific assessments according to protocols and procedures. Clinical competence in procedural skills, treatments and patient/caregiver education related to wound care management and acts as a patient advocate in delivery and coordination of patient care.
5. Provides efficient and effective care with the guidance of the physician during the clinic sessions Able to manage a number of patients at the same time.
6. Collaborates with other health care providers, wound care center physicians, Clinical Program Director and medical director, regarding clinic and patient needs.
7. Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served. Able to interpret data about the patient?s status in order to identify each patient age specific needs and provide care needed by the patient group.
8. Ability to revise plan of care as indicated by the patient?s response to treatment and evaluate overall plan for effectiveness.
9. Ability to adequately assess and reassess pain. Utilize appropriate pain management techniques. Educates the patient and family regarding pain management.
10. Demonstrates ability to perform treatments and provide services to level licensure.
11. Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate.
12. Knowledgeable of medications and their correct administration based on age of the patient and clinical condition.
13. Consults other healthcare professionals as appropriate to provide interdisciplinary approach to the patient?s needs.
14. Documentation meets current standards and practices.
15. Attends seminars, conferences, continuing education classes in the areas of wound prevention, wound management. Participates in professional societies. Stays current with changes, new advances/treatment in wound care.
16. Operates all equipment correctly and safely.
17. Demonstrates an ability to be flexible, organized and function under stressful situations.
18. Inputs charges and enters documentation on all wound care patients into computer system in a timely manner to ensure compliance.
EDUCATION:
Graduate of an accredited school of nursing with current Wyoming licensure as a Registered Professional nurse. Prefer Bachelor of Science in : Minimum of two (2-3) years? experience in medical/surgical, home health, or wound care. Certified Wound Care Nurse (CWCN) preferred. Must be well motivated, organized and able to prioritize multiple tasks. Must be able to interact effectively with a wide variety of people. Excellent communication skills. Able to serve as an example to other employees.
