Patient Case Manager Job Opening in Portland, Oregon - Marquis Mt. Tabor in SE Portland is looking to hire a Patient Case Manager for their Post-acute Rehab!
In the Patient Case Manager role your nursing skills will be utilized as you conduct and coordinate the development and completion of the resident assessments (MDS) in accordance with current rules, regulations, and guidelines, including the implementation of CAAs (Care Area Assessments). As a Patient Case Manager you will also coordinate with Physicians (orders-delete), other healthcare providers, facility inter-disciplinary teams, families and manage the care of our patients through the Charge Nurse, Certified Nursing Assistant and Certified Medication Aide to best meet their quality of life and care needs.
The qualified candidates will be a RN with some long-term care experience and a strong passion for working with the senior population. Experience with the MDS process, case management, and/or being a nurse supervisor is strongly desired. 3+ years' experience working as a RN is required. You will receive excellent, hands-on training to learn Marquis nursing systems and procedures. Our organization also uses an electronic medical records (EMR) system and you will receive extensive training to use the EMR.
