Patient Care Coordinator Job Opening in Versailles, Missouri - At Katy Trail Community Health , we?ve created an approach to health care that revolves around the health of our patients. We are seeking a full-time Patient Care Coordinator with excellent customer service and communication skills. The Patient Care Coordinator must believe that healthcare should be personal, accessible and affordable. The Patient Care Coordinator works with a health care team to address barriers to care which may affect patient compliance with their care plan. Barriers to address may include, but are not limited to financial barriers, transportation assistance, language barriers or other concerns regarding cultural appropriateness.. Some duties include:
Follow up with patients/families who have not kept important appointments, such as rechecks, preventative care or post hospitalization
Participate in providing formal and informal education based on community needs
Manage the Patient Assistance program
Assist patients with the enrollment of medical coverage such as Medicaid and Affordable Care Act (Market Place)
Requirements:
Must have a high school diploma or equivalent.
Knowledge of health-related financial assistance, including medicare and medicaid preferred
Knowledge of available community resources and the ability to evaluate resources and make appropriate referrals preferred
Ability to interpret and comply with federal, state, and local laws and regulations
Able to deal effectively with a diverse patient population
Demonstration of good communication and computer literacy skills
