The Patient Care Coordinator, with the supervision, support and guidance of clinic physicians, supports the development, implementation and success of the Patient Care Coordination program by working with patients, staff and other local support teams. The Care Coordinator will serve as the primary interface between the clinic, individual primary care physicians and their patients and health plan sponsors.
Qualifications:
The successful candidate will possess flexible thinking, excellent communication skills, the ability to examine, evaluate, and discuss the effectiveness of the care plans. He/she must be flexible and have the ability to work collaboratively with physician, clinic staff and patients, be dedicated to their quality of work, and diligent about performance monitoring.
The successful candidate will understand and have a commitment to the philosophy, mission, values and vision of the clinic. The qualified individual will be able to demonstrate these values with his/her leadership practices and a personal commitment to health and well-being. This individual will also possess both a professional integrity and demeanor, and be a professional representative of the clinic. Strong analytical and decision-making skills, along with considerable tact and diplomacy, are all very important.
High School diploma required
Bachelor's degree preferred
Motivational experience desired
Exceptional customer service experience
Experience working in a physician office environment preferred
Effective use of computer technology: MS Excel, MS Word, Internet
Ability to organize data in support of reporting needs
Presentation creation and data base management skills desirable
Ability to prioritize, administer sound judgment and resolve issues in a timely manner
Flexibility and ability to easily and readily adapt to change
Possess tact, consideration, and effective communications skills and ability to work with a wide array of people and interact in a positive fashion
Positive/supportive communication style to establish trust in order to create an engaging work environment
Ability to work a flexible schedule to meet the needs of all stakeholders involved
Demonstrates ability to be self-directed, highly organized, multi-tasked capable, and proficient in problem solving skills
Demonstrates ability to work effectively with all levels of administrative and professional personnel
Working knowledge of medical community and resources
Demonstrates teamwork skills and cooperation, sharing resources, negotiation and problem solving
Responsibilities:
Assist physicians in the development, implementation and maintenance of care plans and delivery of coordinated services
Obtain patient registries and other clinical reports to assist physicians in population management activities
Assist the patient in coordination of any additional tests, images and consults with specialists as deemed appropriate by the PCP in developing the care plan
Assist physicians in maximizing all performance, quality and efficiency rewards
Coordinates and enlist clinic staff members to use patient data to improve the health of the clinic?s population
Provides administrative support for integrated care activities at the clinic
