Qualifications
Certified Activities Director
Familiarity with Federal, State and local-term care requirements.
Demonstrates competence in leadership, tactful interpersonal communication, problem solving, and positive customer relationships.
Able to encourage residents to meet their potential
Excellent communication skills, and able to follow verbal and written instruction,
Must function independently, exhibit flexibility, personal integrity and the ability to work effectively with residents, personnel, families and support agencies.
Related healthcare experience and knowledge preferred.
Responsibilities
Plan, schedule and implement a program of individual and group activities based on residents? schedule.
Assess and complete the sections of the Minimum Data Set (MDS) assigned completely, timely and accurately.
Use the resident assessment protocols (RAPs) to determine whether to proceed to care planning for concerns related to a resident?s activity program needs.
Gather data and document required ancillary assessments including but not limited to resident assessment protocol summary documentation.
Identify and document in the resident?s care plan, the activity of interests, needs, and concerns of the resident. Care plans are reviewed and updated as necessary but at least quarterly.
Document all interaction with resident and/or family in the assessment, care plan and progress notes as required by federal and state requirements.
