Position Summary:
As a Wellness Coordinator, you will be responsible for providing client care coordination, completing client assessments, assisting program management with questions regarding healthcare for their assigned caseload, and for providing training to our Direct Support Professionals and other staff.
Minimum Qualifications:
Education: State of Indiana Licensed Practical Nurse or Registered Nurse is required and the license must be active and in good standing. Must be CPR certified.
Work Experience: Minimum of six months of hands on experience related to the job duties and responsibilities. Prefer individuals with experience working with persons with disabilities
General Requirements: Must have a valid driver's license, acceptable driving record, meet agency's driver insurability requirements, reliable vehicle, and up to date automobile insurance.
Working Conditions: This position may require bending, lifting and stooping. Must be able to use hoyer lift or gait belt for client assistance, if necessary during assessments.
Training During Employment: Train the Trainer Certification (CORE A & B) will be required in addition to internal training requirements and professional training that will ensure department compliance with regulations and best practices.
Position Responsibilities:

Maintains the integrity of client health plans through consultation with the program management team, clients and IDTs,
Proactively trains staff on assigned subject areas, assuring competency standards have been met,

Promotes respectful, open and trustworthy relationships with internal and external stakeholders.
Position Details:
Assist in teaching and training staff and consumers. Provide client risk plan training as needed during new hire orientation.
May assist as needed with administration and reading of TB skin tests.
Perform initial consumer admission assessments and admission documentation.
Participate in the development of and evaluation of individual plans of care for area related to health, hygiene, nutrition, risk, etc. under the direction of an RN.
Develop high risk plans as necessary.
Ensure medical and dental needs of consumers are met.
Coordinate services and communication with physicians and pharmacists as needed.
May change dressings using both sterile and non-sterile technique.
Monitor the medication administration of all consumers and assist in education of consumers regarding self-administration of medications.
Practice safety, environmental, and/or infection control methods.
Communicate effectively to staff, consumer family members, other service providers, and other organizations.
Assist with required paperwork. This may include, but is not limited to the following: Concurrent reviews, quarterly reviews, annual reviews, monthly conferences medical review, plan of care and nursing summaries.
Record and report observed symptoms, reactions, treatments, and changes in the patients' conditions and meet documentation standards.
Other duties as needed.
