Community Health Coordinator Job Opening in Southgate, Michigan - The Community Health Coordinator (CHC) will be responsible for providing DIRECT INTERVENTIONS aimed at helping members and their families to navigate and access community healthcare services and to adopt healthy behaviors. The CHC supports clinicians and the Behavioral Health Care Manager through an integrated approach to care coordination and community outreach. As a priority, activity will promote, maintain, and improve the health of a member.

Qualifications:
Education
Bachelor's Degree (or higher) in social work, psychology, criminal justice, or other human services related field.
Experience
Familiarity with behavioral health environments and mental health diagnoses
Licensure
SST, LBSW, LLBSW, LLMSW, LMSW, LPC, LLPC
**You must have a valid Driver's License and be willing to use your own vehicle when working with members in the community (mileage reimbursed).
