We have a new employment opportunity for a RNAC / MDS
Coordinator with a well-respected Long Term Care Facility located in Idaho Falls,
Idaho. This position requires a detail oriented nurse with substantial MDS 3.0 experience and
a desire to ensure quality in medical care for long term care residents. We are looking for a
Registered Nurse (RN) who is familiar with MDS 3.0, care planning, and completing assessments in a
skilled nursing environment.

Full-time or Part-time positions available.

The MDS Coordinator will provide guidance to facility employees pertaining to RAI processes and
their correlation to resident care and reimbursement. This person must carefully follow
revisions to MDS/PPS requirements and sure compliance with current regulations.

Additional MDS Coordinator responsibilities include :
Initiating development of individual care plans and care directives utilizing nursing process,
Minimum Data Set (MDS), Care Area Assessments (CAAs), and resident input.

Providing feedback to corporate leadership on the status of clinical and financial
reimbursement.

Auditing and analyzing MDS data for accuracy.

Identifying areas for improvement, formulating an action plan, and following through to ensure
progress.

Coordinating and performing nursing assessments at time of admission and ongoing.

Participating in the daily quality review of new admission medical records, physician orders,
and nursing reports for the prior 24-72 hours.

Updating resident care plans and care directives as necessary in compliance with
regulations.

Utilizing the MDS scheduler to plan and setting MDS Assessment Reference Dates (ARDs).

Conducting interdisciplinary resident care conferences at 72 hours post admission, during the
Admission RAI process, upon inter-Center unit transfer, and quarterly or at time of a significant
change in condition per regulations.

Maintaining the Medicare Prospective Payment System (PPS) communication board
