Community Support Team-Team Lead Job Opening in Durham, North carolina - Sunrise Clinical Associates, PLLC is seeking for a Community Support Team-Team Lead.
Qualifications:
Licensed Professional who is an individual who holds a license, provisional license, certificate, registration or permit issued by the governing board regulating a human service profession. Licensed Professional who has the knowledge, skills, and abilities required by the population and age to be served. Associate Level licensed professional actively seeking licensure may serve as the team leader conditional upon being fully licensed within 30 months from the effective date of the DMA Clinical Coverage Policy #8A. For Associate Level licensed team leaders hired after the effective date of the DMA Clinical Coverage Policy #8A, the 30-day month timeline begins at date of hire.
Job Roles, Duties and Responsibilities
Provides individual therapy for beneficiaries served by the team.
Provides behavioral interventions such as modeling, behavior modification, behavior rehearsal.
Designates the appropriate team staff so that specialized clinical expertise is applied as clinically indicated for each beneficiary.
Provides and coordinates the assessment and reassessment of the beneficiary?s clinical needs.
Provides clinical expertise and guidance to the CST members in the team?s interventions with the beneficiary.
Provides the clinical supervision of all members of the team for the provision of this service. An individual supervision plan is required for all CST members.
Determines team caseload by the level of acuity and the needs of the beneficiary served.
Facilitates weekly team meetings of the CST and document on template.
Ensures the monthly Adult Team Meetings are held and documented.
Monitors and evaluates the services, interventions and activities provided by the team.
Provides psycho-education as indicated in the PCP (regarding the identification and self-management of the prescribed medication regimen, with documented communication to prescribing practitioner(s).
Assists with crisis interventions.
Assists with the development of relapse prevention and disease management strategies.
Coordinates and oversees the initial and ongoing assessment activities.
Develops the initial PCP and its ongoing revisions and ensures its implementation.
Monitors and documents the status of the beneficiary?s progress and the effectiveness of the strategies and interventions outlined in the PCP.
Generates appropriate documentation of services provided and submit in a timely manner as stipulated in agency?s policy and Records Management and Documentation Manual.
Communicates the beneficiary?s progress and the effectiveness of the strategies and interventions with team.
Participates in Utilization Reviews and other Clinical Meetings when necessary.
Monitors continuously to ensure titration occur (when a beneficiary needs more or fewer services, the team provide services based on that level of need).
Ensures NC TOPPS is completed during required intervals, as referenced in the NC TOPPS Implementation Guidelines.
Ensures selected modality is utilized during service delivery.
Submits required paperwork on or before established timelines set by the agency.
Adheres to the following: personnel, Health & Safety, Compliance Plan and other agency guidelines.
Attends required trainings to keep abreast of agency, LME-MCO and Divisions? regulations, guidelines and procedures.
Participates in client chart audits and other QA/QI projects.
Consistently demonstrates appropriate discharge and/or transition planning throughout service delivery.
Ensures incident reports are submitted within deadlines stipulated by agency LME-MCO and NC DHHS.
Completes 90 day and annual performance evaluation for assigned team members.
Performs other managerial duties, tasks and assignments delegated by supervisor or Executive Management staff.
Submits documentation for weekly Intensive In-Home team meetings, monthly Child & Family Team Meetings, monthly supervision logs and client specific clinical outcomes.
NOTE: This position must be credentialed by Alliance Behavioral Healthcare and Cardinal Innovations Healthcare MCOs.
