Home Health RN Case Manager Job Opening in Meridian, Idaho - Abode Healthcare is seeking compassionate and dependable RN?s to join our home health team in the Boise area.

It is our goal to provide quality care to our clients in their own homes to minimize the effects of illness, accident, or disability. Performs on-call responsibilities and provides on-call service to patients/families, as assigned.
The Registered Nurse (RN) will instruct patients and their families in proper treatment and helping individuals improve their health. The Registered Nurse (RN) will work under the care plan of the physician to treat the patient.
Responsibilities include:
Performing assessments
Start of care paperwork
Appropriate documentation
Building rapport with patients and family
Initiating evaluations for other services as necessary (PT, OT, HHA, etc.)
Complete supervisory visits with HHA?s
Organize daily assignments
Plans of care updates as necessary
Participate in QA initiatives
EDUCATION and/or EXPERIENCE
Graduated from a National League of Nursing accredited practical or vocational school of nursing.
Unencumbered license as a Registered Nurse in the state that care is delivered.
1 year of nursing experience required; two years nursing experience preferred. Home health or medical/surgical experience preferred.
Comply with the state nurse practice act.
Comply with accepted professional standards of practice.
High level of interpersonal skills.
Certified in CPR.
Strong clinical skills.
Excellent customer service skills and ability to deal tactfully with clients, other team members and community.
Self-directing with the ability to work with little direct supervision.
Licensed driver with automobile liability insurance.
Negative TB test or chest x-ray.
