This position coordinates the care for cancer patients, their family and/or caregivers providing a comprehensive assessment and facilitating timely referrals. Assists cancer patients to anticipate, identify and overcome barriers within the healthcare system. Provides education, support, advocacy, and resources to facilitate informed decision making, as well as providing timely access to quality health and psychosocial care through all phases of the cancer care continuum. Must be a graduate of an accredited school of nursing and possess a BSN. Must be licensed with LSBN. Oncology certification through by ONCC in Oncology (ONC®) , Breast Care (CBCN®), Navigation certification through Academy of Oncology Nurse and Patient Navigation (AONN+) or National Consortium of Breast Centers (NCBC) preferred or completion within two years.
