Affinity is currently looking for a Patient Compliance Specialist (MA or LPN) to work from 8:00AM ? 5:00PM, Monday through Friday.
Job Summary: The Patient Compliance Specialist (MA or LPN) will indirectly help the patient manage their health by reviewing/managing electronic medical records (EMR) and working alongside the patient?s provider. The Patient Compliance Specialist will manage EMR so that all health data systems meet the of government agencies and accrediting professional organizations.
Essential Duties & Responsibilities:
Responsible for maintaining AHG patients? compliance with Disease Management, services, , referrals, tests ordered, and follow-up
Maintains confidentiality of all patient health information data
Ensures that all patient medical information and records are entered into the computer database
Checks regularly with physicians and other healthcare providers to make sure the information recorded is correct and/or complete
Coordinates with non-AHG healthcare providers to forward any medical information regarding a patient
Responsible for contacting patients to ensure that they are following the guidelines of care chosen by the patient and their healthcare provider
Compiles, processes, and maintains medical records of hospitals, non-AHG clinics, and other medical facilities consistent with medical, administrative, ethical, legal, and regulatory requirements
Keeps detailed patient records such as treatment plans, test results, diagnoses, medical history, and description of symptoms
Responsible for contacting other healthcare facilities to schedule patient tests/procedures
Processes, maintains, compiles, and reports patient information for health requirements and standards
with AHG clinic nursing staff and AHG providers regarding patient care
Assist with any new Population Health program initiatives that may arise. Such as, scheduling HRA visits and assisting the Population Health Nurse Practitioner during the visit.
Marginal Duties:
Occasionally performs other duties and responsibilities as assigned
Knowledge, Skills, and Abilities:
Graduate of accredited Medical Assistant (MA) school or graduate of accredited Practical Nursing (LPN) school
2+ years of experience in the medical field preferred
Ability to work with limited supervision and to make decisions based on established policies and procedures
Exceptional interpersonal, verbal, and written communication skills
Customer service oriented attitude and behavior
computer and computer systems skills
Experience operating inside electronic medical records
Ability to effectively communicate with clerical and management personnel within the work site
Supervisory Responsibilities:
None
Supervision Received:
Works under occasional supervision of the Population Health Manager and general supervision of the Population Health Supervisor and Patient Compliance Coordinator
Working Conditions:
Overtime will be occasionally required
Travel may be required but is reimbursed accordingly to our travel policy. See travel policy.
