Program Coordinator II Job Opening in San Jose, California - Community Health Partnership (CHP) was established in 1993 as a non-profit (501c) member organization representing non-profit community health centers in Santa Clara and San Mateo counties. CHP?s current membership is comprised of nine Community Health Centers providing service at 40 sites, combined. Our mission is to advocate for affordable and accessible health services for our diverse and multicultural communities regardless of socioeconomic, ethnic, religious, or cultural background, and to support our member organizations in achieving these goals.
POSITION OVERVIEW - The Program Coordinator II supports the management team in the development, implementation, evaluation, and analysis of a variety of projects involving community health centers. The Project Coordinator II may function as a project lead on more than one health improvement initiative that aim to expand and strengthen the capacity of community health centers.
DUTIES AND RESPONSIBILITIES
Supports strategies to achieve project goals and objectives .
Develops and maintains plans for carrying out project activities.
Plans, coordinates, prioritizes and monitors projects activities.
Plans training and education curriculum, and coordinates training for member agencies.
Collects and maintains data and reporting on key measures.
Builds and maintains relationships with community partners.
Provides technical assistance to member agencies.
Prepares presentations, reports and correspondence.
Conducts an ongoing analysis of current and relevant information related to the projects.
Provides administrative support to ensure compliance with varies reporting requirements.
Attends and participants in community-partner meetings.

Education & Knowledge
Knowledge of theories, principles, and concepts acquired through the completion of
Bachelor?s Degree in Public Health, Healthcare Administration or related field.
Demonstrate an understanding of the impact of social issues in healthcare (Social Determinants of Health).
Knowledge of Healthcare Reform and the current political environment.
Work Experience
Minimum five years? experience in program planning, development and evaluation.
Minimum two years? experience in a non-profit or other healthcare or government setting.
Experience and knowledge of quality and process improvement.
Knowledge of health education and promotion principals and practice.
Must be able to work in a team-based setting.
Willing and able to travel to program related activities.
Demonstrated proficiency using Microsoft Office and manual or web-based applications for data.

Fluency in Spanish a plus.
Must possess a valid CA driver?s license and proof of insurance.
Excellent verbal, written and interpersonal communication skills.
Must be able to pass a criminal background check prior to employment.
