JOB SUMMARY An RN Case Manager develops, implements, and evaluates individualized pediatric patient care plans. They serve as a liaison between patients, their families, and healthcare providers.
REQUIREMENTS
1+ years? experience as a RN
Home health experience a plus!
Pediatric experience a plus!
Valid CO RN License
Valid driver?s license and auto insurance
RESPONSIBILITIES
Work with new pediatric patients, assessing their conditions and needs in order to develop personalized care plans.
Regularly evaluate these plans of care and update them on an as-needed basis.
Educate their patients and their families on how to follow their care plans.
May also provide nursing care to their patients.
