The Care Coordinator position is responsible for providing patients with appropriate referrals to internal/external resources and agencies, coordinate specialty care for clinic patients, schedule appointments for clinic patients with a provider of physician, serve as a liaison between the Provider, Registered Nurse (RN), Licensed Vocational Nurse (LVN), and Care Team Supervisor to ensure patients receive appropriate care. Must interact in a positive manner with staff, patients, families, and members of the community.
ESSENTIAL JOB FUNCTIONS:
Overview:
Completes routine reports, messages, and forms
Communicates in written and verbal form, using correct grammar
Submits all correspondence within requested time frame
Relays all messages as instructed
Organizes and files records according to established clinic procedures
Files records in chronological and alphabetical order
Effectively utilizes office machines to include computer, fax and photocopier
Collects and distributes mail in a timely manner, and in accordance with established clinic procedures
Participates in huddles
Schedules appointments
Takes messages for the Care Team
Follows all policies and procedures
Sends authorizations and referrals, completes and documents follow-up on authorizations and referrals to outside agencies, as determined by medical staff to ensure continuity of patient care
Other duties as assigned by Supervisor
EDUCATION AND QUALIFICATION REQUIREMENTS:
Medical Assistant Certification REQUIRED
High School Diploma or GED required
2-4 years of medical office experience in an out-patient clinical setting.
Must have demonstrated skill in filing, use of copier equipment
Excellent organization and communication skills with attention to detail skills
Bilingual in Spanish REQUIRED
