Sunlife Home Care is hiring for a Clinical Liaison. Must have experience in home health or private pay care giving services, BSN in nursing or MSW in Social Work understand Home Care and care giving services and there demands who is organized, motivated to excel and has a desire for excellence. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Organizes and conducts marketing presentations, educational seminars, community support groups, patient and family initial consultations about services available. The CL will demonstrate exceptional verbal and written communication and presentation skills with individuals and groups.
Customer Service - Community Liaison will ensure extraordinary customer service to both external and internal customers. External customers would include referral sources, patients and their families. Internal customers would include supervisors, clinical & non clinical staff members, pharmacies, and suppliers.
Facilitates resolution of problems between department and referral sources as evidenced by reports and satisfied referral sources.
Promptly notifies Director of any concerns or problems related to referral source satisfaction.
Promptly follows up on referral source concern calls within 4 hours as evidenced by documentation.
THE COMMUNITY LIAISION PROVIDES COORDINATION BETWEEN THE REFERRAL SOURCE, ADMISSION COORDIANTOR AND THE CLINICAL TEAM.
Develops open lines of communication between department and referral sources.
Promptly follows up on inquiry calls within 4 hours as evidenced by documentation.
CL communicates with referral sources regarding clinical appropriateness and recommendation regarding admission.
This position keeps the Clinical Supervisors and Director informed of referrals and pending admissions.
Keeps informed of patient's progress by attending hospice team meetings.
Marketing calls to doctors' offices ? drop off brochures, information, copies of patient's satisfactions, customer service call, education, new programs etc. (weekly)
Calls to hospitals and Rehab Facilities to make contact with the social service department and discharge planners in Counties served (one county per month)
Visit Senior Housing complexes to drop off brochures and educational information.
Assistance with Marketing Blitz during regular scheduled work hours.
Community educational or health events are covered and/or coordinated by the Community Liaison. Some health events will fall after regular work hours in the evening or on weekends.
The Community Liaison will preplan weekly sales calls by Friday for the following week and shared with the Director.
Prepares monthly report of market calls made and follow-up information with a copy to the Director of Home Health & Hospice at the end of the month.
Attends and/or coordinates special events. This includes scheduling space, sending out invitations, planning programs, speakers and refreshments.
PERFORMS QUALITY ASSURANCE AND PERFORMANCE IMPROVEMENT ACTIVITIES.
Reviews patient surveys monthly.
PERFORMS OTHER DUTIES AND SPECIAL PROJECTS AS REQUIRED.
Assistance to be supplied on other projects and duties as needed.
PERFORMS ADDITIONAL DUTIES AS ASSIGNED, WITHIN THE SCOPE OF RESPONSIBILITY
