RN Case Manager:
The Case Manager will assess, develop and implement individualized care plans that address all physical, psychosocial, environmental and safety factors so that our clients remain as independent as possible.
Preventing hospitalizations or re-hospitalizations is a fundamental priority.
Each Case Manager maintains a small caseload of clients (6) in which he/she is responsible for coordinating all aspects of the client's care services.
Includes interacting with the client, client's family, physicians, therapists and support services.
Job Requirements
Registered Nurse in the State of Iowa
Ability to demonstrate superior assessment and technical skills
Previous hospital, nursing home, or home health care experience
Holds a valid Iowa Driver's license
Is self-directed and has the ability to work with minimum supervision and exceptional organizational and time management skills.
