(HPI), a private non-profit corporation, is one of the largest freestanding behavioral healthcare agencies in the area. HPI, accredited by the Joint Commission and licensed by the State of Michigan, provides a broad array of mental health and substance abuse treatment and prevention services to individuals of all ages.
HPI is seeking an SBIRT Case Manager for the Emergency Department at Garden City Hospital.
LOCATION : Garden City Hospital ? Emergency Department, Garden City, MI
SHIFT: Part-Time; Monday (7:00 a.m. - 5:30 p.m.) and Saturday (8:00 a.m. - 6:30 p.m.)

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS
The Screening, Brief Intervention, and Referral to Treatment (SBIRT) Case Manager must be able to work in a multidisciplinary team in medical and dental settings. The SBIRT Case Manager shall have, at minimum, a Bachelor's degree from an accredited college or university, specializing in social sciences (i.e. Social Work, Psychology). The incumbent must obtain or maintain current, appropriate issued by the State of Michigan, including Certification through Michigan Certification Board for Addiction Professionals (MCBAP). The incumbent must have at least 1 year of direct professional social service/case management experience, of which 3 months was spent working with adults with substance use disorders, within the past 3 years. The incumbent should at minimum be able to demonstrate understanding of screening and knowledge of the identification and treatment of co-occurring mental health and substance abuse disorders. Applicants should be able to demonstrate basic knowledge of diagnostics, psychopharmacology, and supportive treatment approaches as applied to a SUD adult population. Applicants must have a working understanding of Motivational Interviewing and have skill with engaging consumers to participate in SBIRT process.
ESSENTIAL JOB RESPONSIBILITIES
Technical
Complete Alcohol Use Disorder Identification Test (AUDIT-C) on all patients.
Complete AUDIT and/or Drug Abuse Screening Test (DAST-10) on all patients that screened positive on the AUDIT-C.
If SBIRT at Hope Clinic, complete the Patient Health Questionnaire - 9 (PHQ 9) and Generalized Anxiety Disorder- 7 (GAD-7) scales during screening.
If SBIRT at Hope Clinic, must follow screening for depression and anxiety policies and procedure and discuss with primary care provider at Hope Clinic.
Determine the appropriate level of intervention for patients based on the scores of the AUDIT and DAST-10 (and based on the PHQ 9 and GAD 7 scores if at Hope Clinic).
Complete the necessary Pre-Admission for all patients who received an AUDIT-C.
Complete discharge in Electronic Medical Record system (Avatar) if patient did not screen positive
Complete admission process in Avatar for all patients who did screen positive, including completion of intake and progress notation.
Complete progress notes for all contact made with the patient.
Provide Brief Intervention and Brief Negotiated Interview for all patients who scored in the range appropriate for this level of intervention.
Assist the patient identify and document goals related to recovery and wellness, if needed.
Contact patients 10 days after initial contact and document a progress note.
Provide Brief Treatment to patients that qualify and document progress of this service in Electronic Medical Record System.
Assist the patient with referrals, contacting supports, payor sources, and community resources and document in the Project Fit Activity log.
Assign a Peer Recovery Coach for all patients who qualify for a Referral for Treatment.
Complete Discharge once patient has been active for 6 months or per the patient request.

Assist in building a high-performing SBIRT team.
