Role Description
The Risk Adjustment Coder is a vital member of the Population Health Coding Department at Oak Street Health. S/he champions the organization's mission to provide high-quality patient care by providing measurable, actionable feedback to providers that will result in improved documentation accuracy and coding practice. The role will be asked to collaborate with doctors, coders, facility staff and a variety of internal and external personnel on a wide scope of Risk Adjustment and Quality education efforts.
Responsibilities
Review medical record information on both a retroactive and prospective basis to identify, assess, monitor and document claims and encounter coding information as it pertains to Hierarchical Conditional Categories (HCC)
Assess adequacy of documentation and query providers to obtain additional medical record documentation or to clarify documentation to ensure accurate and appropriate coding
Audit provider documentation of CPT, ICD-9 and ICD-10 codes to ensure adherence with CMS Risk Adjustment guidelines
Evaluate HCC auditing processes and provide analyses and recommendations to improve overall provider documentation and coding
Maintain and grow the current knowledge of the Medicare and Commercial Risk Adjustment billing systems/processes

What we?re looking for

Ability to work independently with minimum supervision, excellent reliability, and a positive attitude
CRC (?Certified Risk Coder?) credential highly favorable
Medicare Risk Adjustment (HCC) and ICD10 coding experience is required.
Strong communication skills
Strong computer skills and basic knowledge of Microsoft Excel
US work authorization
