Home Healthcare/ Hospice LPN Utilization Review Nurse Job Opening in Estes Park, Colorado - The LPN utilization review nurse is responsible for ensuring that clinical records are reviewed and processed appropriately. Reports To: Clinical Director
Duties 1. Received case referrals. Reviews available patient information related to case, including disciplines required, to determine home care needs. Assigns appropriate clinicians to case. 2. Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services.
3. Reviews patient's clinical diagnosis, medications, procedures and clinical course.
4. Conducts ongoing record reviews and communicates findings and recommendations to clinical director and appropriate organizational personnel.
5. Processes clinical records through the appropriate computer systems for state submission and validation.
6. Visits patients in the home for skilled care as needed.
7. Complies with accepted professional standards and principles.
8. Promotes customer service orientation to all organization personnel.
9. Performs other duties and activities as delegated by the Clinical Director.

Qualifications 1. Licensed practical nurse with current licensure to practice professional nursing in the state. 2, Complies with accepted professional standards and practice.
3. Complies and maintains current CPR certification.
