Practice Manager Job Opening in Bennington, Vermont - The Practice Facilitator for the Bennington Blueprint for Health Grant serves as a coach and consultant to physician practices to prepare for the National Commission on Quality Assurance's (NCQA) Patient Center Medical Home Recognition, to support on-going performance improvement and practice transformation, and to advise on medical home requirements related to team based care, panel management and population : Bachelor's degree in healthcare, performance improvement or management preferred. Training in Clinical Microsystems and/or other performance improvement methodologies required. Three years experience in performance improvement, practice facilitation, or project management strongly preferred. Three years in leading projects and/or working independently with meeting defined goals. Experience in working with physicians preferred. Valid Driver's license.
