Care Coordinator Job Opening in Richmond, Virginia - Summary
The Care Coordinator will be responsible for supporting patients and their families in managing high blood pressure, adopting healthy behaviors, and accessing community services and other resources. This role offers additional support to the Commonwealth Primary Care (CPC) Care Coordinator through a team-based approach to care management at the clinic sites. Specific Care Coordinator roles include providing social support and informal peer coaching, advocating for individuals and community health needs, and providing services such as conducting health risk assessments and surveys, and follow-ups. Care Coordinators will also connect patients to community resources and supports.
Job Responsibilities
Responsible for establishing a trusting relationship with patients that include families and natural support systems, while providing general support and encouragement
Coordinating and assist with scheduling patient appointments at CPC
Conduct intake interviews with patients to assess their needs
Create or utilize tracking mechanism to follow patient?s adherence to established goals
Follow up with patients via phone calls, text messages and emails
Help patients set personal goals and decrease barriers to ensure adherence
Help patients coordinate transportation to appointments (if needed)
Follow up with health management/care plans with both patients and providers
Coach patients to effectively self-manage and control their blood pressure using best practice resources including a Blood Pressure Self- Management Toolkit
Work collaboratively and effectively within a healthcare team
Establish positive, supportive relationships with patients
Assist patients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible
Overcome patient barriers to obtaining needed medical care and social services
Exhibit excellent working relationship with patients, visitors and staff
Facilitate communication and coordinate services between providers
Motivate patients to be active, engaged participants in their health
Effectively work with people (staff, clients, doctors, agencies, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions
Become familiar with the policies, procedures and resources of the provider/health system as it relates to Care Coordinator project duties
Identify and apply appropriate role definition and skilled boundaries
Advocate for participants internally and externally in the community
Attend regular meetings
Adapt to processes and procedures instituted by the Richmond City Health District and clinics
Minimum Qualifications
Bachelors in a health field or experience in social work, care coordination, or other related health experience
Community Health Educator, LPN, RN
Written and oral fluency in English
Flexibility in work hours and travel within the Richmond Metro-Area
Knowledge of the Richmond community being served
Basic computer, phone, and/or tablet skills
Basic knowledge of medical terminology or health literacy
Driving required to clinic locations in Henrico and Richmond City multiple days of the week
Clean driving record
Background check
Ability to establish and maintain positive working relationship with care team and other organizations.
Excellent Interpersonal Communication Skills
Ability and willingness to provide emotional support, encouragement, and motivation to patients.
Ability to work both independently and within a healthcare team
Classification
Full time, contract position, funding currently available for one year August 2017 ? July 2018 (with possibility to continue) $20.00/hour
