Responsibilities
The RN Case Manager plans, organizes, and directs home health and hospice services.
The Registered Nurse is responsible for evaluating patients.
The Registered Nurse plans, implements, and documents nursing care based on patient's needs.
Overall responsibility for the development and management of the plan of care for the assigned case load.
Patient population includes pediatric, adult, and geriatric patients.
Average case load of 10 - 14 patients*

Qualifications
Nursing Degree
Current RN licensure in the State of Idaho
Problem solving skills and ability to multi-task
Compassionate with good communication skills
Excellent teamwork skills
Valid state driver's license
Current automobile liability insurance
Dependable transportation
