Outpatient Coordinator Job Opening in Providence, Rhode island - This position is responsible for managing and coordinating care for a panel of high intensity members with low individual will facilitate appointments for the member, which include Health Plan and Prospect?s programs as related to their diagnosis, PCP, Specialist, High Intensity clinic and Home Provider appointments and transportation if needed.
Job
Tracking the member?s compliancy with physician orders and evaluating PCP or Specialist access issues.
Participate in patient education regarding urgent care, hospice, Care-Plus clinics, flu vaccination and other issues related to Chronic Care management.
Generating letters, faxing to PCP and mailing to members along with resources. In addition, documenting, tasks, opening and closing cases for new and termed members.
Maintaining a panel of non-complex members Notify Care-Plus case manager when members are due for re-assessment Processes referral request for High Intensity program members( CareRx, NursePractitioner, Palliative Care, Wound and Behavioral Health)Maintain turn-around-time compliance with all referrals.
Assist Case Managers in obtaining medical records for Care-Plus members and forward them to the nurses or Home Visiting Providers.
Qualifications
Minimum Education: High School diploma or Associate degree preferred.
Minimum Experience: Knowledge of managed care and healthcare preferred. Type 60 wpm. Proficient in MS Office programs(i.e., Word, Excel, Outlook, Power Point)Req.
