The Pharmacy Coordinator is responsible Coordinating and assisting clinical pharmacist
To perform medication reconciliation post-hospital discharge to reduce hospital re-admission rates, hospital bed-days, ER visits and total healthcare costs. Maintain an atmosphere of professionalism and confidentiality.
Job
Schedule patient appointments for medication reconciliation consultations. Process referrals for medication reconciliation. Communicate and obtain pertinent information for consultation from patients, physician offices and case managers. Document notes in the appropriate tracking system. Produce necessary reports on a regular basis. Assist patients and prescribers with any barriers in obtaining medications. Provide support to clinical pharmacists as part of disease management processes.
Make home visitations to gather necessary information needed for medication reconciliation through a face time with a pharmacist.
Qualifications
Minimum Education: High School graduate or equivalent.
Minimum Experience: Prior work experience in pharmacy setting (i.e. retail pharmacy) or managed care setting is required. Bilingual in Spanish preferred. WPM -50 (minimum). Working knowledge of MS Word and Excel
Req. : Active pharmacy technician license required (PTCB/national pharmacy technician license preferred)
