SOCIAL WORKER/CASE MANAGER
The Sisters of Mercy, a community of Roman Catholic women, are dedicated to helping those in need through education, advocacy, healthcare, housing, pastoral and social services. The Northeast Community with administrative offices in Cumberland, Rhode Island is currently searching for a full time Social Worker/Case Manager.
Reporting to the Director of Long Term Care and Retirement, the successful candidate will be a passionate social worker providing services to our Sisters in the Northeast. Primary office is based out of Cumberland, Rhode Island.
Specific responsibilities include, but are not limited to:
Visits with Sisters in their homes, community offices, assisted living facilities, nursing homes and hospitals.
Conducts comprehensive assessments to identify Member?s needs.
Interacts with and assists the Local Coordinators with housing and health care needs of members.
Initiates and oversees timely applications of members for specific government, health care and housing programs in collaboration with Local Coordinators and Healthcare Coordinators.
The successful candidate must be able to work independently and as part of a team. Strong oral and written skills, good interpersonal skills, strong organizational skills, the ability to multitask and solid working knowledge of Microsoft products including Outlook, PowerPoint, Excel, etc. are expected. In addition, the final candidate will hold a Bachelor?s in Social Work with a minimum of 2 years? experience in a medical support setting such as home care, hospital, assisted living or nursing home servicing elder clients with interest and experience in a geriatric social work and case management are required. Valid driver?s license a must.
