Case Manager II - H3OK Program Job Opening in Oklahoma City, Oklahoma - SUMMARY
Provides advocacy, linkage, referral, case management, and independent living skills training to clients who require frequent contact in order to maintain community tenure, by performing the following duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Adheres to agency policy and procedures, as found in the Forms Locked drive on each computer. Also adheres to CARF and DMHSAS standards and criteria, federal and state confidentiality rules and regulations, and any special funding source requirements including Medicaid and HCA regulations as well as HUD.
Supports and promotes a service environment for individuals in need of behavioral health and/or co-occurring services that is recovery focused, welcoming, and attentive to the needs of individuals who may have experienced trauma in their lives. Attends annual core competencies training in relationship to individuals with co-occurring disorders or who have experienced trauma in their lives.
Establishes and maintains professional working relationships with supervisor and peers. Can effectively work within a team or group setting cohesively.
Monitors eligibility status and assist eligible consumer in applying for Social Security, Title XIX (Medicaid) or QMB, and other benefits for which the client may qualify.
Maintains up-to-date address, phone number, contact persons, and financial information on all clients.
Establishes rapport with clients, assessing client needs; organizes, plans and delivers face-to-face client services; and conducts follow up on the outcome of services provided.
May be required to complete best practice screenings to determine eligibility for programs, provide orientation to program, and maintain contacts for clients awaiting entry into housing programs.
Coordinates the needs of clients and family members with resources available and provides client advocacy as needed.
Assists clients in transitioning from a higher intensity level of services to lower levels of intensity other programs.
Provides outreach, engagement, and case management to homeless veterans and chronically homeless non-veterans. Coordinates the needs of clients with resources available and provides client advocacy as needed.
Provides intensive services for clients to assist in maintaining outpatient recovery and utilizes the individual assessment to determine the frequency of services.
Assists clients in finding and accessing community resources needed to maintain independent functioning.
Assists clients, individually and in groups, with development of independent and community living skills; promoting client strengths and encouraging self-determination and empowerment which will encourage community involvement and integration.
Assist clients in developing an individualized recovery plan that focuses on personal and interactive problems, independent living skill needs, defining goals, and to plan action reflecting interests, abilities, and needs.
Responsible for maintaining strictest confidentiality guidelines for interactions regarding consumer and family information.
Provides a minimum of 85 hours billable services with consumers per month.
Responsible for seeking out supervisory assistance for additional training and to enhance skill set as needed.
Will provide training for consumers members in performance of responsibilities and duties assigned and ensures each consumer has a realistic understanding of responsibilities as needed to maintain successful engagement in the community living programs.
Interacts with other professionals to discuss therapy or treatment, new resources or techniques, and to share information. Maintains an active and collaborative role within the community and with stakeholders to outreach to homeless veterans and chronically homeless non-veterans.
Maintains accurate rent calculations, grant update information, and completes financial reports as directed by the accounting office in a timely manner.
Reports and documents services in accordance with agency policy and procedures. Adheres to HOPE policy and procedure of having documentation completed by the end of the next business day.
Collect, report, and maintain accurate data needed for the purposes of the program and enters this data in appropriate reporting location such as electronic medical record, HMIS, PICIS, etc.
Follows the fidelity of models utilized in provision of services and participates in fidelity monitoring as required by grant or funder.
May be required to work evenings or weekend hours.
May be required to provide services in a community setting.
May be required to provide client transportation using personal vehicle or agency owned van or small bus.
Performs other related duties as assigned.
QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE
Bachelor's degree in psychology, social work, or other related mental health field.
CERTIFICATES, LICENSES, REGISTRATIONS
Required to obtain and maintain DMHSAS CM Certification
Required to obtain and maintain First Aid and CPR Certifications.
Required to complete CAPE, Therapeutic Options, or approved safety training and annual updates.
In order to provide client transportation in private or agency vehicles, must possess current, valid Oklahoma license, and meet HOPE's vehicle insurance requirements. HOPE will annually obtain copy of Motor Vehicle and Record from the State Department of Public Safety.
LANGUAGE SKILLS
Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence and clinical in medical records chart. Ability to effectively present information in one-on-one and small group situations to families, clients, and other employees of the organization.
REASONING ABILITY
Ability to apply commonsense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Ability to seek out supervisory assistance for additional training when needed and make application of techniques or other skills to meet essential job duties and responsibilities.

Ability to relate to person with severe mental illness and/or substance use in a positive, helpful and constructive manner.
Ability to establish and maintain effective and tactful working relationships with co-workers; to express ideas and facts clearly and concisely in written or oral form.
