We are a non-profit organization providing dental clinic services, prevention, education and outreach via our Garden Grove clinic, Smile Mobiles and CHOC hospital location. We are looking for a full-time Outreach Care Coordinator to join our team! The ideal candidate will have at least one year of community dental/medical healthcare experience. Position Summary :
The Outreach Care Coordinator under the supervision of the Community Wellness Manager works with the Outreach Program to help facilitate dental services to families with school aged children. The Care Coordinator will link families to dental homes and community resources. This position is to support and oversee the utilization of services from initial contact at community and school events, through screenings, to the establishment of a dental home and completion of services.
Essential Duties:
Maintain and oversee case load of clients accessing dental services at clinical and community sites.
Provide patient care coordination which includes: parent follow up calls, provider calls, scheduling of appointments, and access to transportation, insurance application assistance, and translation services as needed.
Maintain data collection in Excel and create reports of care coordination case load.
Complete data reports on a monthly basis and as needed.
Track outcome data for care coordination which includes linkages to a dental home, initiation of treatment, and completion of treatment.
Liaison for referrals between the community dental provider partners and agencies for general and specialty dental services.
Additional responsibilities:
Working with the community dental provider partners to facilitate access to services for children and their families.
Attend monthly, quarterly community meetings identified by the Community Wellness Manager.
Provides information to target populations about oral health through culturally sensitive, age and developmentally appropriate activities.
Actively supports other members of the Outreach team and the organization
Other tasks as identified by the Community Wellness Manager and/or VP of Operations.
Work area locations are determined by supervisor; either Garden Grove or Santa Ana offices.
Position Qualifications:
High school diploma or GED.
1 year of community health center (medical/dental) clinic experience.
Proficiently able to speak and write English and Spanish.
Proficient in Excel.
Ability to plan, prepare, and present health related materials, in a clear and concise manner with minimal supervision.
Ability to communicate effectively and respectfully with individuals from different racial, ethnic, cultural and social-economic backgrounds.
Demonstrates knowledge and sensitivity to issue/needs of the children and their families.
Experience working with one or more of the following: families, community groups, nonprofits, health programs, children 0-17.
A valid California driver?s license, own transportation.
Ability to travel up to 15% around Orange County.
