Patient Care Coordinator - Remote Job Opening in Stuart, Florida - Medical Billing Group is a rapidly growing healthcare revenue optimization company. We are looking for a Care Coordinator (CNA, CMA, LPN) to deliver Chronic Care Management (CCM) and support the onsite Care Coordinator with CCM, eligiblity, and scheduling acitivities. Candidate MUST reside in the local Stuart, Florida area.
The Care Coordinator will be responsible for:
Telephonic communication with the patients to achieve the following:
Enroll patients in Chronic Care Management program
Create Care Plans specific to their chronic conditions
Review and update the Care Plan with the patients
Medication reconciliation
Educate patients on healthy living habits and dietary plans based on their conditions
Schedule phone calls
Maintain supporting documentation to enable payments from Medicare
Document all patient activities in the CCM & AWV platform
Notify provider of any changes in care plan or out of range data points
Required Skills and Qualifications:
Certified Medical Assistant, Licensed Vocational Nurse, or Licensed Practical Nurse
Excellent communication (phone) skills
Ability to and experience with working from home
Ability to effectively interact, in both oral and written forms, with staff, physicians, patients, and families
Operational knowledge of MS Office (Word, Excel, PowerPoint) and Adobe Acrobat
Detail-oriented with excellent organizational skills
Ability to meet productivity standards with minimal supervision
Salary range based on experience $18-23/hour
