Role Summary:
Provides diabetes self-management education services to patients and/or families including assessment of nutritional status, diabetes/nutrition care planning, counseling and evaluation. Interacts with other health care team members for planning and delivering appropriate patient care.
Key Responsibilities:
Provides quality diabetes self-management education based on client expectations and needs.
Conducts a thorough individualized, age-specific assessment.
Develops an individualized interdisciplinary diabetes care plan with short- and long-term goals.
Communicates, coordinates, monitors, and documents the plan of care.
Assesses the outcomes of diabetes intervention and revises the plan as appropriate.
Uses age appropriate learning methods, appropriate strategies and reinforcement techniques.
Creates an educational setting conducive to learning with adequate resources to learning with adequate resources to facilitate the learning process.
Demonstrates understanding of stages of behavior change and ability to counsel individuals on these stages through development of behavior change goals.
Assesses patient readiness to learn and patient perceived needs.
Facilitates experiences that assist the patient in achieving identified goals.
Updates educational materials to assure accurate, current and scientifically sound content.
Documents to standards of Think Diabetes Education Program using Allscripts (EHR).
Additional Responsibilities:
Acts as mentor to all staff in area of specialty.
Effectively applies evidence-based research to enhance practice.
Seeks out and reviews research findings in their application to practice.
Bases practice on sound scientific principles, generally accepted standards, research and theory.
Assures clinical care in area of diabetes specialization is state of the art.
Systematically evaluates the quality and effectiveness of practice and revises practice as needed.
Participates in routine review of program outcomes.
Serves as a member of the Think Diabetes Advisory Board.
Complies with all federal and state
Adheres to the Academy of Nutrition and Dietetics Code of Ethics if a Registered Dietitian.
Participates in special projects, committee assignments, and program development.
Assumes responsibility for professional development and continuing education to acquire current knowledge and skills.
Required Training:
All applicants must possess a valid Certified Diabetes Educator (CDE) credential in accordance with the minimum qualifications set by the National Certification Board for Diabetic Educators (NCBDE).
Required Education and Experience:
Bachelor's Degree in Nursing, Dietetics, Clinical Nutrition, or related healthcare field from an accredited college or university.
Minimum 2 years experience practicing diabetes education in a clinical setting
