With over 16 programs located across Western Wayne County's communities, Hegira Programs, Inc. (HPI), a private non-profit corporation, is one of the largest freestanding behavioral healthcare agencies in the area. HPI, accredited by the Joint Commission and licensed by the State of Michigan, provides a broad array of mental health and substance abuse treatment and prevention services to individuals of all ages.
IMPORTANT NOTICE: Prefer LMSW, LPC, or LLP for this position. Limited License Clinicians can apply, however you must have at least 1 year of experience working with adults with chronic mental illness.
Minimum Education and Experience Required
The Community Intervention Services Therapist shall have, at minimum, a Master?s Degree from an accredited college or university, specializing in psychology, social work, or guidance and counseling.
He/she shall obtain and/or maintain current, appropriate issued by the State of Michigan. In addition to documented educational training,
The Community Intervention Services Therapist shall have provided direct treatment services to adults in a behavioral health inpatient or outpatient setting for not less than 18 months, of which at least 6 months was spent working with adults with co-occurring disorders, within the past 3 years. In addition to direct work experience,
The Community Intervention Services Therapist shall, minimally, demonstrate (1) basic knowledge of diagnostics, psychopharma­cology, and supportive treatment approaches as applied to a severely mentally ill (SMI) adult population; and (2) knowledge of the identification and treatment of co-occurring mental health and substance abuse disorders.
He/she shall have the ability to accurately read maps and driving directions and must also possess a valid Michigan chauffeur's license with less than six current points.
LOCATION : Adult Outpatient Services Facility, Westland, MI
SHIFT: Full-time, Afternoons; schedule must include at least two Saturdays and one Sunday per month.
COMPENSATION: $15.25/hr - $16.50/hr depending on experience

ESSENTIAL JOB RESPONSIBILITIES
Technical:
Intake
· Formulate preliminary DSM five-axis diagnosis.
· Identify issues that may exist regarding safety (personal and environmental), natural supports, and community involvement.
· Identify referrals needed for comprehensive treatment, including medical, nutrition, substance abuse, STD/HIV, education, and vocational training.
· Obtain releases of information for coordination of care, including primary care physician, past treatment episodes, emergency and significant support persons, court, and school related contacts.
· Educate patient and patient?s parent/guardian with respect to admission information packet, including but not limited to, Medicaid Grievance procedures and Recipient Rights processes.
· Demonstrate knowledge of patient payor source.
· Complete Urine Drug Screen, as indicated.
· Schedule appointment for psychiatric evaluation within appropriate time frame relative to medication supplies and/or acuity of symptoms.
Treatment Planning and Progress Reviews
· Complete an individualized Treatment Plan for each patient by the fourth session or within 30 days, whichever is first.
· Utilize the patient?s Person Centered Questionnaire in development of treatment plan and support person participation.
· Treatment Plan problem areas, goals, and objectives address the patient?s DSM five-axis diagnosis and areas identified on the bio-psychosocial assessment, including but not limited to, personal and environmental safety, relapse or decompensation potential, child and family strengths, and individual needs.
· Demonstrate use of natural supports in Treatment Plan.
· Complete Treatment Plan Reviews (TPR) minimally after the first 60 days of treatment and 180 days thereafter.
· Update Treatment Plan via a TPR following a change in clinical status including, but not limited to, change in diagnosis, hospitalization, or utilization of crisis services.
· Utilize HPI corporate, continuous Treatment Plan process when receiving or facilitating an internal patient transfer.
· Inform patient of local dispute resolution process at the time of each Treatment Plan and TPR.
Discharge Process
· Document on Treatment Plan criteria and anticipated date for planned discharge.
· Update changed discharge criteria on the TPR.
· Document plan and course of transfer to less intensive level of care on an approved HPI clinical form.
· Discharge patients with no contact for 45 days.
· Provide written notification to Medicaid recipients of intent to terminate services via an Advanced Action Notice.
· Complete Discharge Summary within 21 days of discharge date.
· Complete Patient Discharge Demographics form within 21 days of discharge.
· Complete a Continuing Care Plan for each patient discharge at the last session for planned discharges and within seven days for unplanned discharges.
Ongoing
· Coordination of appointments with psychiatrist and other primary therapists, as applicable.
· Provide linking and coordination of community-based services.
· It is expected that a 55% direct activity level will be consistently maintained.
· Legibly document using language meeting professional standards, abbreviations per HPI approved listings, and error correction processes per policy.
· External documentation is typed on HPI issued letterhead or clinical document.
· Transport patients as needed to community-based supportive, adjunctive services.
· Provide individual supportive, solution-focused psychotherapy not less than once every 30 days to all patients on caseload.
· Provide group therapy, as assigned, demonstrating utilization of supportive and/or psycho-educational approach, based on assignment.
· Start and stop therapy, collateral, and consultation sessions as scheduled, unless crisis intervention services require otherwise.
· Document all patient contacts and/or contacts related to case management services, both attempted and completed, on an approved HPI clinical record form.
· Initiate contact, by phone or in writing, to non-compliant patients within 24 hours of a missed appointment.
· Communicate with hospital staff and/or hospital liaison in the event of an inpatient admission, prior to the patient?s discharge from the hospital.
· Meet with patient in hospital within two (2) working days of notification of hospitalization.
· Ensure a minimum of three contacts with post-hospitalized patients within ten days of hospital discharge, including a minimum of one face-to-face contact within seven days of discharge.
· Complete a progress note for all treatment services that include clinical presentation, a summary of clinical content of the session as it relates to the patient?s identified problems, medication compliance, and progress toward the goals/objectives written on the patient?s treatment plan.
· Assist patients and/or other clinical staff with various case management tasks, including but not limited to, medication compliance, referrals, and linkage to resources.
· Provide 24-hour crisis intervention services to patients, as necessary.
· Each day worked, complete and submit (within 24 hours) to clerical a printed schedule corresponding to patient services documented in each patient?s record.
· Report medication issues, including non-compliance and adverse side effects, to clinic psychiatrist and/or clinic supervisor immediately.
· Consult with clinic psychiatrist and/or clinic nurse within five days of the identification on a non-emergent medical condition.
· Monitor substance use via Urine Drug Screens, as indicated.
· Complete required annual documentation updates and/or referrals, per HPI policy.
· Regularly maintain and clean HPI vehicles.
· Operate any and all company vehicles assigned in order to complete job assignments in a safe and appropriate manner at all times.
Interpersonal:
· Establish effective therapeutic bonds with patients assigned to caseload.
· Maintain therapeutic relationships within boundaries established per licensing guidelines and HPI ethical pledge.
· Maintain collegial, cooperative relationships with peer therapists, other staff of HPI, and community resources/referring agencies.
· Report all Recipient Rights complaints and knowledge of potential complaints to CSS/ACSS within 24 hours of said knowledge.
· Contribute pertinent clinical information at regular case conference meetings.
· Annually attend a minimum of 10 monthly staff meetings.
· Attend weekly Case Conference meeting with CIS team.
· Follow directives provided in staff meetings.
· Respond to written requests from CSS/ACSS or HPI Administration regarding clinical and/or operational information within indicated time frames. Must utilize indicated formats, if applicable.
Critical Thinking:
· Provide services at frequency and intensity consistent with individual patient diagnosis and current clinical status.
· Report to CSS all referred clinical cases that are outside the scope of your expertise, experience, or in which counter transference issues might jeopardize the patient?s clinical outcome.
· Provide CSS/ACSS and psychiatrist with timely, pertinent information regarding changes in patient(s) condition(s).
· Provide CSS/ACSS with timely, pertinent information regarding a patient?s state of mind that has potential impact on the safety of the clinic environment.
· Maintain a broad-based knowledge of referral sources, both internal and external, and make appropriate referrals as needed.
· Advise CSS/ACSS caseload difficulties in advance of scheduled time off.
· Consult with CSS/ACSS in the event a patient presents at risk of harming self or others, either intentionally or otherwise.
· Be knowledgeable of potential side effects or adverse reactions of psychotropic medications.
· Transport only designated Hegira Programs, Inc. patients.
· Ensure compliance with no smoking policy in HPI vehicles.
· Report any and all accidents or vehicle maintenance issues immediately as they occur to the CSS/ACSS and in the event of an accident, immediately initiate an HPI variance report and police report.
· Report and document all behavioral issues pertaining to clients to appropriate CSS/ACSS in a timely manner.
Productivity:
· Adult Population: Maintain a minimum of 55% direct to indirect service ratio.
Physical Requirements:
· Must have adequate visual acuity to interact with the Electronic Health Record System and accurately input data.
· Must have adequate visual acuity to safely follow all driving laws and regulations and possess a valid driver?s license.
· Must be able to give physical assistance, as needed, to clients as they enter/exit the vehicle.
· Must be able to sit up to 50% of work shift.
· Must be able to pass an eye exam.
· Must be able to walk/stand up to 60% of work shift.
· Must be able to endure light to moderate amounts of physical exertion while actively engaging in indoor and outdoor activities with members up to 25% of work shift.
