CareLink Mission: The CareLink mission is to lead its members in the development of a high quality, responsive and relevant aging services network that cares for adults with complex health issues.
Position Overview: The Case Manager is responsible for the coordination, referral, and support of medical, social, and community services for members. The central role of the Case Manager is to provide person-centered, individualized attention to members in need of peer support, care coordination, referrals, and assistance accessing services.
Full-Time position, 40 hours/week, Monday-Friday (8:00am-4:30 pm)
Primary Responsibilities:
Manage a case load of identified and/or assigned members; conduct telephonic and community-based outreach
Provide comprehensive documentation of all activities and contacts in Case Management Software System (Acuity)
Complete Comprehensive Functional Needs Assessment (CFNA) with member in the home or in the community
Provide telephonic initial screen of medical needs, coordinate services and make appropriate referrals
Provide information to members and/or families regarding social and community support; assist with accessing social services as appropriate
Conduct off-site visits (such as but not limited to: home; hospitals/community health centers; or other community agencies) as necessary or required
Provide assistance in obtaining access to care and services; assist member with making and keeping appointments
Recommend and participate in the development of program improvements and Quality Initiative (QI) activities
Participate in Outreach/Marketing activities as needed and appropriate
Maintain up-to-date knowledge of resources in Rhode Island
Participate in afterhours/weekend on-call scheduled rotation
Perform other duties as assigned
Requirements:
Bachelor Degree preferred. In lieu of degree, equivalent experience required.
Knowledge of adult Medicaid population
Strong knowledge of community and state resources; proven history of connecting clients to resources
Experience in person-centered care planning and working with clients to establish goals
Ability to work as part of an interdisciplinary team and actively participate in team meetings and case conferences
Excellent communication skills and the ability to interact respectfully with culturally and linguistically diverse populations
Experience working with geriatric/chronic illness, people with behavioral health and/or substance abuse issues
Self-motivated, commitment to improving access to health care for underserved population
Ability to multi-task, prioritize tasks to meet deadlines, problem solve and work effectively in a fast ?paced growing environment
Ability to work independently and maintain good judgment and accountability
Regard for confidential data, adherence to organizational policies/procedures as well as HIPAA
Bilingual (English/Spanish) a plus
Valid RI Driver License and access to reliable transportation; travel throughout RI required
