The Patient Advocate will support the efforts of the clinical team and will contribute to the processes necessary for a patient centered medical home. This Patient Advocate will be under the supervision of the Director of Nursing & Access who is responsible for improving the quality of patient management, promoting continuity of care, and providing coordinated care for NTACHC patients. The Patient Advocate will be responsible for establishing and maintaining positive relationships and coordinating care within the clinic, external service organizations, agencies, and other healthcare facilities in the community. The Patient Advocate is also responsible for listening to, understanding, and translating spoken or written statements from one language to another.

Responsibilities
Assists in chaperoning of patients for procedures with the direction of a physician.
Explains treatment procedures to patients in the patient?s language when requested and as necessary
Prevents complaints by counseling with supervisors and managers who see a problem developing; offering suggestions to resolve potential complaints; modifying practices that cause repeated complaints.
Documents complaints by listening to patients and their families; directing them to a provider or supervisor; helps the patient present facts to the clinic representative; developing acceptable resolutions; following up with outcomes.
In coordination with Director of Nursing and Access, champions the patient satisfaction implementation on a monthly basis
Supports and promotes patient health education initiatives in collaboration with key personnel

Working knowledge of standard concepts, practices, and procedures.
Able to work efficiently and cope with emergency situations.
Capable of using experience and judgment to plan and accomplish goals.
Good computer skills and a working knowledge of Microsoft Office.
Able to read and interpret documents such as charts, safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence.
Professional manner and appearance.
Emotional and physical health sufficient to meet the demands of the position.
Ability to stand for long periods of time.
Minimum Qualifications
Bilingual in English/Spanish - REQUIRED
1-2 years of experience working in a clinical setting ? PREFERRED
Experience with NextGen Healthcare System or other Electronic Medical Record system - PREFERRED
Must be computer literate and have exceptional telephone and computer system skills
Ability to communicate effectively with patients and all levels of personnel in a professional, courteous and effective manner using excellent customer service skills
MUST possess excellent communication and organizational skills with the ability to multi task, prioritize, and meet deadlines.
