SUPERVISOR: Behavioral Health Manager
The Substance Abuse Community Health Worker role is to coordinate and provide access to substance abuse community resources; facilitate communication among all providers; empower patients to be active participants in their healthcare.
DUTIES:
Comprehensive case management
Build partnerships with community organizations, service providers, and other entities to work collaboratively in support of BNHC activities.
Referral to community and social support services
Comprehensive transitional care across health care organizations
Individual and family support
Consultation and co-management in the treatment of mental disorders, substance use, and psychosocial issues particular presenting cultural perspectives
Interpretation as needed by patient
SLILLS:
Experience with Substance Abuse services preferred
The ability to establish rapport quickly and to communicate effectively with patients, their family members and community providers.
The ability to function effectively as a member of an intra-professional team that includes behavioral health and primary care providers, patients, and family members
Excellent knowledge of community resources for mental health, behavioral health, and other social service needs
The ability to create and implement integrated care plans, ensuring access to an array of linked services, and the exchange of information among patients, family members, and providers
Language and cultural diversity knowledge (Cape Verdean Creole, Portuguese, Haitian Creole, French, and Spanish)
Requirements :
Bachelor?s Degree in Social Work or related field
Local travel as necessary
