Person Centered Care (PCC) Specialist - Full Time Job Opening in Napa, California - Collabria Care is comprised of a team of dedicated staff and volunteers committed to providing exceptional professional healthcare, education and service to community. We are committed to providing exceptional professional health care, expert resources and compassionate support to individuals and their loved ones experiencing the transitions of aging, learning to live with chronic illness, or facing end of life.
Our Day Program,is seeking a full time PCC Specialist to assist participants with personal care, toileting, meal service, activities and other duties as directed by the RN Supervisor.

* Certified Nursing Certificate, a High School Diploma and Training in Personal Care Assistance required.
* A minimum of one (1) year work experience in providing personal care with elderly persons and disabled adults preferred.
* Possession of a valid California Drivers License and the ability to maintain automobile insurance.
* Ability to meet requirements of CPR, First Aid and other mandated programs
