SUMMARY While under the clinical supervision of a licensed mental health professional (MHP), the Mental Health Paraprofessional (MHPP) works with clients who have a wide variety of physical, cognitive, social, emotional, developmental or age-related needs to help to improve their independent functioning as prescribed on the treatment plan. The MHPP observes and documents client responses to TR/RT interventions. ESSENTIAL DUTIES AND RESPONSIBILITIES ? Provide all services and functions within Alternative Opportunities Code of Conduct. ? Adhere to all company policies for employment. ? Coordinate care and participate with the clinician in the treatment planning process by conducting functional assessments. ? Understand the key values and activities of case management. ? Demonstrate understanding of overall nature, course, and treatment of mental illness. ? Demonstrate understanding of mental health service systems and assist program participants in identifying, securing and sustaining a range of resources needed to facilitate independent community living. ? Assist in formulating a treatment and rehabilitation plan that will ensure that needed services are delivered. ? Conduct problem-solving groups to assist consumer in addressing challenges. ? Serve as an advocate to acquire services for consumers. ? Coordinate family involvement in the treatment and rehabilitation process. ? Evaluate services that are provided to ensure that necessary improvements are made to increase outcomes. ? Provide the continuity of contact for the program participant throughout the entire service process. ? Coordinate access to housing, social services, substance abuse treatment and other community resources, and consults with these resources to assist persons in achievement of desired goals. ? Help program participants negotiate the system of financial entitlements to ensure they have adequate income and healthcare coverage. ? Understand and respond to consumer rights and advocate for program participants. ? Understand vocational rehabilitation services in a manner that facilitates access for program participants. ? Develop supports for program participants who are employed or involved in employment programs. ? Teach program participants how to maintain employment through solving problems that arise on the job. ? Assist program participants in daily and independent living skills. ? Monitor and access program participants? behavior and functioning and provide therapists with information relevant in supporting treatment plan development. ? Serve as a conduit and link between program participants and the community services and resources. ? Assist program participants in learning appropriate social skills and behaviors. ? Assist program participants with linkage to family members and family support. ? Serve as a member of a multidisciplinary team, supporting the development of a plan of care that is based on individual strengths, needs, abilities, and preferences. Page 2 of 2 MHPP; Revised 02.2015 ? Conduct functional assessments to aid in the development of the individual treatment plan. ? Participate in organizational processes, as directed by the program coordinator, which may include quality assurance, peer record review, and health/safety activities. QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. EDUCATION and/or EXPERIENCE Bachelor?s degree from an accredited college or a high school diploma and documented experience in the area of mental health services is required. Staff are required to successfully complete 40 hours of prescribed classroom training within two (2) weeks following employment or demonstrate that they have received the MHPP certification training while working for another company. CERTIFICATES, LICENSES, REGISTRATIONS MHPPs are required to successfully complete 40 hours of prescribed classroom training within 2 weeks following employment or demonstrate that they have received the MHPP certification training while working for another company and maintain MHPP certification throughout the duration of employment in this position. A valid driver?s license, current auto insurance, reliable personal transportation and an acceptable driving record and are required.
