Patient Care Manager Job Opening in Patten, Maine - Katahdin Valley Health Center is now hiring a Patient Care Manager for our Patten location. The Patient Care Manager is responsible for managing high risk chronic illness patients to promote effective education, self management support and timely healthcare delivery to achieve optimal quality and financial outcomes. The Patient Care Manager will work as part of the Patient Care Team to insure effective and quality care is given to the KVHC patient.
Graduate of an accredited school of nursing. Current licensure as a Registered Nurse by the State of Maine. Minimum of two years professional experience in a related field. Previous work experience in patient care management preferred. Experience in healthcare administration.
