Wellness Coordinator Job Opening in Plymouth Meeting, Pennsylvania - Position Summary:
The Wellness Coordinator will work under the direction of the Wellness Supervisor to assist in the planning, implementation and evaluation of health promotion plans aimed at improving the health status of the membership.
Core responsibilities for this position include conducting a lifestyle assessment, developing an individualized health promotion plan, establishing goals and formulating a strategy to address members? individualized needs. The Wellness Coordinator will also serve as a liaison within the local community to identify and secure resources for programming needs.
The work of the Wellness Coordinator is reflective of the ideals and values of the organization including integrity, simplicity, and respect for the inherent dignity of every human being and fostering the independence of each member.
Responsibilities:
Conduct initial home visit with new members in order to:
a) establish a relationship with the new members; b) acquaint them with the operation of the Care Coordination Department; c) establish a contact schedule; d) gather required personal information; and e) establish a baseline of information regarding the member?s health, support system, and layout and safety of home dwelling. f) review results of the lifestyle assessment tools as completed by the members.
Maintain contact with assigned members and/or their families based on the applicable member contact schedule.
Using the lifestyle assessment tools, assist members? in creating individualized health promotion plans. Guide members in accessing resources to help achieve their wellness goals. Resources may include the VigR Enrichment Workshops, the Mastery Experience, the eMeetinghouse web community, the Health Resource Library and various community health and wellness resources.
Follow-up and monitor members? individualized health promotion plans. Provide advocacy and health education on an ongoing basis.
Provide consultation to other Care Coordinators based on credentials and/or specialized expertise (e.g. nursing, social work, occupational therapy, hospice, etc.).
Understand and comply with all applicable policies, procedures and standards.
Understand and comply with all aspects of continuing care agreements with members.
Understand and be able to arrange for and be able to oversee the provision of care to members.
Demonstrate proficiency in data entry and comply with all data entry requirements.
Submit reports as required.
Maintain confidentiality of member/client information and comply with the HIPAA regulations and standards.
Provide on-call coverage during evenings, weekends, and holidays on a rotating basis with the other coordinators.
Participate in educational program planning as required for quarterly Member meetings.
Serve on committees/work groups as appropriate or requested.
Understand and comply with all applicable policies, procedures and standards.
Perform other related duties as assigned.
Qualifications:
Degree in Social Work, Nursing, Psychology or other Health and Wellness related disciplines with a minimum of 3 years of experience working in related health care field.
Must maintain credentials and state licensure as appropriate and required.
