Care Coordinator - Le Mars Job Opening in Le Mars, Iowa - Plains Area Mental Health Center is seeking a Care Coordinator in our Le Mars office to provide services to members. IHS is a model of healthcare delivery that utilizes a patient-centered holistic approach to coordination of care for the Medicaid eligible mental health population. The Care Coordinator is responsible for making and tracking referrals, follow up on referrals, completing assessments, and coordination of behavioral health professionals for adults with a Serious Mental Illness. Must have a Bachelor?s degree in Social Work or a Human Services field. Travel required.
