Home Care Coordinator Job Opening in Lansing, Michigan - Safe Hands Warm hearts Inc. is a locally owned home care company currently seeking highly motivated Home Care Coordinators. The Home Care Coordinator is responsible for the monitoring of client care supervision of caregivers to ensure services are provided as stated individual?s service plans.
Responsibilities:
Work with caregiver staff to ensure total care is provided to individuals based on the care plans
Co-ordinate with nurses, pharmacies and care manager to deliver timely medications
Creating monthly schedules
Report to management on areas of improvement in care delivery
Monitor and record any caregiver mistakes during the delivery of care
Demonstrate a passion for learning and professional development
Observing clients' health status, taking actions to address concerns, and reporting all significant changes
Train caregivers and substitute staff on all aspects of direct service provision and implementation of the service plan
Establish positive internal/external working relationships
Share pertinent information with necessary parties in a timely manner. Use respectful, open and honest communication
Work actively to identify and remove barriers to services
Learn from mistakes, admit mistakes, and take personal responsibility for decisions and actions of self and staff
Give constructive, timely feedback.
Visit, revisit, inform and educate between 15 and 20 network referral sources on a weekly schedule
Assist Safe Hands Warm Hearts Inc. in developing, managing and maintaining referral relationships
Acknowledge all network referral source contacts with appropriate correspondence
Continually research and identify sources for client referrals
Maintain records & provide the Manager with weekly networking activities and progress report
Provide advice to management on key developments within the service area
Keep up to date on information and issues related to the position, the community, and referral networks
Participate in internal and external professional development activities as appropriate
Requirements:
Minimum of 3 years of CNA experience, management experience preferred
Preferred CNA/HHA certification
Should have strong networking, presentation, and follow-up skills
Self-confident, enthusiastic, strong initiative and motivation, and a desire to succeed
Ability to work independently and handle multiple tasks concurrently
Must have reliable transportation and current valid auto liability insurance
Must be self-directed and have the ability to work with little supervision
Outstanding customer service, teamwork, communication skills, and initiative
Demonstrate caring and compassion nature along with the ability to handle client needs
Clear background check, drug test, Tuberculosis (TB) test
Must be 18 years of age and able to work independently
Must enjoy working with senior population
Job Context:
The position entails a high degree of accountability and consequence of error.
The position represents the organization in many different forums. Professionalism in dress and conduct as well as appropriate representation of the agency is expected.
This position requires communication and interplay with a number of internal and external stakeholders. Therefore, professional and timely communication is considered a key component of this position.

The position requires work in the evenings and weekends.
The position requires the performance of tasks requiring visual, manual, communication, and analytical abilities within an independent work setting.

Salary: $28,000.00 to $32,000.00 /year
